This assignment is designed to help you reflect on your mental health clinical experience while applying the nursing process, ethical principles, therapeutic communication, and evidence-based nursing care. The goal is to consider how nurses advocate for and support clients experiencing mental health challenges and their families.
Assignment Instructions
Write a 500-word reflection paper based on one of the following experiences:
· Your mental health clinical rotation
· A Med/Surg or other clinical patient with psychosocial or mental health concerns
· An approved movie option listed in D2L (only if you have not yet attended your mental health rotation or cared for an appropriate patient)
Your paper should be organized using the following headings: You MUST use the Template provided!

4. Nursing Intervention Rationales & Evaluations

· PART 1. Introduction and Nursing Diagnoses (Approx. 125 words)
Provide a brief introduction to your client without using identifying information. Include:
· Age range and relevant background information
· Psychiatric diagnosis/diagnoses
· Pertinent physical health diagnoses
· Clinical setting or situation
· Then identify your top two priority nursing diagnoses from your clinical observation sheet.
(Requirements for Nursing Diagnoses)
· Include:
· One 3-part nursing diagnosis
· One “Risk For” nursing diagnosis
· Nursing diagnoses must be written in correct NANDA format.
· “Risk For” diagnoses should not include evidence because the problem has not yet occurred.
· Include Rationale
Briefly explain why you selected each diagnosis using:
· Client observations
· Patient statements or quotations
· Staff or family reports
· Behavioral findings
· Assessment data
PART 2. Reflection (Approx. 250 words)
Reflect on how nurses can provide the best possible care for this client and their family.
Address the following questions:
